Discounts do not apply to the Preconferences. To qualify for the group discount, all registration information must be complete for all group members, payment included and forms sent together. If faxing, list all group members on cover sheet. Subtract the discount from the tuition fee. Groups may not be formed after individual registrations are received.

Partial Attendance (Online registration is not available)
To register, call Contemporary Forums for tuition and CE credit for individual days. Group discounts do not apply. Registrations may not be shared. 

Cancellations
Cancellations must be made by phone or in writing. A $75.00 processing fee will apply. No refunds will be issued for cancellations received or postmarked on/after June 24, 2017. However, tuition paid, less the $75.00 processing fee, may be applied to a future Contemporary Forums conference of equal or greater value. No refunds for "no shows."

Thank you for registering for the Pharmacology for Advanced Practice Clinicians conference, being held July 24 - 30, at the Hyatt Regency San Francisco, 5 Embarcadero Center, San Francisco, CA 94111, (415) 788-1234.

PRECONFERENCE CHECK-IN

Preconference Monday, July 24 at 7:00am. You must check in to receive your conference materials. The location of the Registration Desk will be listed on the hotel's event schedule.

MAIN CONFERENCE CHECK-IN

Tuesday, July 25 from 7:00 - 8:00 a.m. If you attended the Preconference, you do not need to check in again for the Main Conference.

SESSIONS

Begin promptly as listed in the conference brochure and online and are open only to registered participants. Out of consideration to others, infants, children and spouses are not permitted in the meeting rooms or refreshment break areas.

RECORDINGS

Audio recording, video recording and livestreaming of speakers, sessions and/or attendees is strictly prohibited.  Medical information presented at this conference is intended for educational purposes only and should not be considered medical advice.

ATTENDEE ACCOUNT ACCESS

Valet parking cost is $59 +$8.26 tax per day with in and out privileges.
Self parking option is available near the hotel at a lower cost without in and out privileges. Daily maximum rate (24 hours) is $33.

Fare from San Francisco International Airport to the Hyatt Regency is approximately $45-$55. Taxis are available outside the baggage claim area.
