Preconference A: Diabetes Bootcamp for Primary Care Providers: Tips from the Experts

This Preconference will focus on increasing the attendees’ knowledge of diabetes to effectively manage this chronic disease.  The presentations from the experts will focus on individualized treatments to target specific pathophysiology in order to achieve glycemic control.  They will also discuss insulin and non-insulin treatments, how to improve pregnancy outcomes in diabetic women, and the advantages of technology in diabetes monitoring and management.  Review of case studies will demonstrate how primary care providers can apply this knowledge to clinical practice.

  • Discuss the latest concepts in diabetes etiology, progression and targeted treatment.
  • Describe and define how to initiate insulin and non-insulin treatments for the diabetic patient.

Preconference B: Introducting Comprehensive Treatment for Obesity into Primary Care Practices

Obesity is now recognized as a chronic disease with significant co-morbidities.  It is epidemic in the United States and clinicians in primary care settings must be at the forefront of understanding the complex nature of treating this disease in order to assist patients in managing this chronic disease.  Over the last four years, new medications have come onto the market, which can be added to the tools available to support patients in managing obesity and meeting their goals of a healthier life.  This workshop is designed to review the pathophysiology as well as the current evidence for treatment of obesity as a prominent chronic disease including appropriate referrals and billing.  Join your colleagues at this opportunity to work through case studies and apply the information learned during the presentations.

  • Recognize the epidemiology and pathology of the chronic disease of obesity.

Method of Tuition Payment
Make payment by check (payable to Contemporary Forums and enclose with Registration Form), Visa, MasterCard, Discover, American Express or Purchase Order. A $30 fee is charged for any returned check.Personal checks are NOT accepted at the conference. Purchase Orders must be received with registration by the conference date and include the P.O. number, invoice address, tuition, approval signature, contact name, email address and phone number. 

Group Discounts (Mail, fax or call in together. Online registration is not available)
Tuition discounts are given to groups of 3 or more registering for the complete Main Conference.
Discounts do not apply to the Preconferences. To qualify for the group discount, all registration information must be complete for all group members, payment included and forms sent together. If faxing, list all group members on cover sheet. Subtract the discount from the tuition fee. Groups may not be formed after individual registrations are received.

$25 per person discount for 3 or 4 in a group
$45 per person discount for 5 or more in a group

Partial Attendance (Online registration is not available)
To register, call Contemporary Forums for tuition and CE credit for individual days. Group discounts do not apply. Registrations may not be shared. 

Cancellations
Cancellations must be made by phone or in writing. A $75.00 processing fee will apply. No refunds will be issued for cancellations received or postmarked on/after February 3, 2017. However, tuition paid, less the $75.00 processing fee, may be applied to a future Contemporary Forums conference of equal or greater value. No refunds for "no shows."

Program Changes
Contemporary Forums reserves the right to make necessary changes in speakers, topics or schedule. The most current program can be found on our website.

Thank you for registering for the Chronic Disease Prevention & Management in Primary Care conference, being held March 2 - 5, at the Hard Rock Hotel and Casino, 4455 Paradise Road, Las Vegas, NV 89169, (702) 693-5000.

PRECONFERENCE CHECK-IN

Preconference A: Thursday, March 2 at 7:00 a.m. Preconference B: Thursday, March 2 at 12:15 p.m. You must check in to receive your conference materials. The location of the Registration Desk will be listed on the hotel's event schedule.

MAIN CONFERENCE CHECK-IN

Friday, March 3 from 7:00 - 8:00 a.m. If you attended the Preconference, you do not need to check in again for the Main Conference.

SESSIONS

Begin promptly as listed in the conference brochure and online and are open only to registered participants. Out of consideration to others, infants, children and spouses are not permitted in the meeting rooms or refreshment break areas.

RECORDINGS

Audio recording, video recording and livestreaming of speakers, sessions and/or attendees is strictly prohibited.  Medical information presented at this conference is intended for educational purposes only and should not be considered medical advice.

ATTENDEE ACCOUNT ACCESS

As a registered attendee, access your account to the conference by clicking HERE, click on My Account to the right of Return to Calendar and enter your First Name, Last Name, Email and Zip Code.  Please ensure that the information you enter matches your registration information.

Once on your account, you will access the following for viewing and downloading:

1. E-Conference Guide, which is available about 5 days prior to the conference. 

2. Speaker Slides and Handouts, which are available, provided that the speakers turn them in, 5 to 10 days prior to the conference. 

3. Session Evaluations, made available during the conference after the session has ended.  Evaluations must be completed within 30 days after the completion of the entire conference.

4. Certificate of Attendance, available upon completing all speaker and conference evaluations.  A copy in PDF format will be made for viewing and printing.
