Conference Program
Sunday, October 14
PRECONFERENCE A
Emergency Department Case Management Services: Taking Center Stage in Healthcare Transformation Introduction: Departments?
Join case management leaders to study Emergency Departments (ED), the fastest-evolving area in health care, as they address the changing needs of their surrounding communities while tackling increasingly complicated clinical, psycho-social, quality, and financial issues. Some hospitals and their EDs continue to be challenged to determine the appropriate level of care for patients.
Others have moved beyond to the successful use of observation units, discharge planning directly from the ED, decreasing revisits to the ED, as well as readmissions to acute care, specializing ED rooms for specific populations, and creating strategic care plans with the community and even competing hospitals for patients that are high utilizers and/or have complex medical and behavioral problems. This Preconference will cover rules, roles, regulations, and the all-important ROI (return on investment) as well as the latest innovations and predictions for ED care and case management.
Objectives Part I
- Identify rationale, roles, functions, and competencies of the ED RN case manager and medical social worker in relation to the EDs physicians, multidisciplinary staff, and special stresses.
- Describe lessons learned from implementation experiences in a wide range of hospitals, including rationale, FTEs, scheduling, orientation, handoffs, use of physician advisors, and hospital politics.
- Review functions directly related to ED Case Management, such as access, transportation, bed placement, observation units, billing, software, and CMS regulations.
Objectives Part II
- Evaluate the current and morphing role of EDs in the US society, economy, complex patient populations, and specialty EDs.
- Participate in a national survey of current practice and predictions about the structure and function of EDs.

PRECONFERENCE B
Case Management Administration: What Are Experts Doing to Manage
Complex Case Management Departments?
Introduction:
Hospital case management departments are at the core of the clinical and financial functions within a hospital. It is increasingly demanding to manage or direct a case management department as the regulations surrounding health care are changing so rapidly. The one constant in case management is change and the challenges are complex. Failure to navigate these changes can be financially catastrophic to your organization. This Preconference will be filled with fun, interactive, fast-paced, and dynamic sessions on some of the most challenging topics that case managers face today. The day will culminate in an Open- Space session for sharing ideas and experiences. This interactive session will assist you in developing a Change Package to take back to your organization to implement the valuable ideas and best practices you have identified.
Objectives
- Identify the roles, structures and staffing models of some of the most successful case management departments.
- Describe strategies to manage, display and communicate critical case management data to key stakeholders.
- Identify 3 goals physicians, hospital administration, and case management departments have in common.
- List and recognize specific case management components of both the OIG Work Plan and Hospital Conditions of Participation.
- Identify at least one strategy/driver, key change concept, and action item to test your "Change Package."
